Former apprentice Yoji Harada, now a full-fledged member of the design crew, joins Ami in doing simultaneous tattoos for a pair of best friends. One customer gets a cartoon version of herself, and another asks for a Buddha design to help cover up a scar from scoliosis. A woman with no tattoos comes in to apply for the shop manager position.

Bei Miami Ink herrscht Chaos. Um die Dinge wieder in den Griff zu bekommen, stimmt Ami Núñezes Plan für ein neues Studio zu, und Darrens überraschende Rückkehr von einer längeren Reise trägt zur guten Stimmung bei. Allerdings fehlt ihnen immer noch ein Tätowierer und sie brauchen einen neuen Studioleiter. Garver reist nach Costa Rica, um ihren alten Freund Tim Hendricks zu überreden, zu Miami Ink zu kommen. Ami und Núñez versuchen, Baby Dre, eine lesbische Marokkanerin, davon zu überzeugen, von LA zurück nach Miami zu ziehen, um das Studio zu leiten. Inmitten des Chaos erkrankt ein Tätowierer an einer unangenehmen Hautkrankheit, ein mysteriöser Geruch macht sich im Studio breit, die Crew macht einen Roadtrip und einige erleben einschneidende Veränderungen in ihrem Leben. Trotz des ganzen Trubels läuft der Betrieb weiter. Zu den Kunden gehören eine Domina, ein Veteran, der im Krieg beide Beine verloren hat, ein Mann, der seine Frau und sein ungeborenes Kind durch Krebs verloren hat, und eine Braut, die vor der Hochzeit geflohen ist und ihr neues Leben feiern will.
