On the hunt for a robbery homicide suspect, the police shake down all young black males they encounter, igniting racial tensions throughout the city. The suspect in question turns out to be Puppet, whose younger brother was killed as a result of Puppet's gang activity despite Doc's attempts to turn him away from the streets. After he's shot by the cops and goes into hiding, Puppet solicits Doc's help by proclaiming his innocence. The ensuing lengths to which Doc goes to get Puppet to the hospital and save his life while avoiding the cops wreak havoc with Doc's relationships with both civilians and the squads.

Joey starts a fire while on a field trip with his class, causing Kim and Jimmy to reevaluate their parenting styles and inspiring Kim to try to rebuild her family, starting with a reconciliation with Jimmy. But Jimmy's relationship with Brooke is not nearly as wobbly as Kim would like to think, and Kim ends up jumping into bed with a stranger and then commiserating with her mother over her problems with men, ending in a confrontation with Brooke that forces Kim to rethink what's best for Joey in the matter.

Davis rescues a drowning newscaster from a downed helicopter, and the ensuing rush of media coverage on his heroism brings a strange young woman to meet him. She turns out to be his half-sister, and Davis uncovers secrets about his father's second family. The turmoil from the revelations causes disturbances between Davis and nearly everyone close to him. Meanwhile, someone's throwing bricks at random citizens, putting everyone in the city on edge.

The deaths of four teenagers in a fiery car crash profoundly affects the third watch, who were forced to stand by helplessly, unable to save the victims. They congregate at the end of the shift to dispell their sadness and anger at the horrific images of the crash. Faith, Sully and Doc spend time sitting quietly in a cafe; Ty, Bobby, Carlos and Alex go to a bar; and Kim, Bosco and Jimmy go bowling. As evening turns into day, Faith's group meets up with a young woman, Ty's group meets up with a young man, and Kim's group meets up with a young couple in love. At dawn, everyone ends up at the a beach and they build a bonfire. As the flames flicker, we see a flashback to the moments immediately preceding the car crash and discover that the young people whom they met during the course of the evening are the spirits of the teenagers who died in the crash, who were sent back down to earth to tell them that the crash could not have been prevented, and they are not to blame for being unable to

While coping with work demands, a recovering alcholic husband and a disturbing visit with her parents, Yokas confronts her long held beliefs and emotions.
A group of prisoners escape from a delivery truck. Yokas and Fred have another big fight in which she accuses him of drinking again. She tells him that she had an abortion. He wants her to leave the house, this makes her stay over at her parents home. However, by the end of the episode, they are back together, she realises that she can't live without him and that she is sorry for accusing him of still drinking.
Meanwhile, Bosco gets a bunch of flowers from a former lover. He smells the flowers which turn out to be poison Ivy. Bosco and Yokas also have a fight when she tells him that she had an abortion. He accuses her of lying to him and he refuses to speak to her, until she forces him to go the hospital to sort out the poison Ivy problem. She regrets lying to him.

Der Selbstmord trifft Faith sehr hart, da sie mit ihren eigenen medizinischen Problemen zu kämpfen hat, während Doc sich Sorgen über die emotionalen Auswirkungen macht, die dieser Fall auf Kim haben wird, obwohl sie immer wieder beteuert, dass ihr Unfall kein Selbstmordversuch war.

Die Auszeichnung
Obwohl seine Kollegen ihn für die Auszeichnung "Sanitäter des Jahres" nominieren wollen, ist der nachdenkliche Doc entmutigt, als er sein unerfülltes Leben betrachtet. Außerdem kümmert sich Doc um einen alten, chronisch betrunkenen Mann und ein junges Mädchen, das in der Badewanne einen Selbstmordversuch unternimmt. Währenddessen unterzieht sich Faith heimlich einem Eingriff, um festzustellen, ob der Knoten in ihrer Brust Krebs ist, und überlässt den ahnungslosen Bosco einem nervösen Polizeianfänger; Kim konsultiert widerwillig einen Anwalt wegen Jimmys Absicht, das Sorgerecht für Joey zu beantragen.

Bosco flieht vor der Gegenwart und seiner Vergangenheit, als er fälschlicherweise des Mordes an einer Frau verdächtigt wird, die eines sexuellen Übergriffs verdächtigt wird. Während Faith versucht, Beweise zu sammeln, um ihn zu entlasten, wendet sich Bosco an seine gestörte Familie, zu der seine alkoholkranke Mutter, sein drogenabhängiger Bruder und sein entfremdeter, missbräuchlicher Vater gehören. Faith beendet ihre Bestrahlung und ist in ziemlich schlechter Verfassung, befolgt aber nicht den Rat von Dr. Case, sich eine Auszeit zu nehmen. Faith kehrt nach ein paar Tagen Pause zur Arbeit zurück. Sie entschuldigt sich bei Bosco dafür, dass sie ihre Krankheit verheimlicht hat, und verspricht, ihn nie wieder auszuschließen. Von Schuldgefühlen geplagt, fängt Doc an, bei jedem Anruf an sich zu zweifeln. Kim erinnert ihn daran, dass Ryan erstickt wäre, wenn Doc ihm nicht den Kragen abgenommen und ihn intubiert hätte. Sie sagt ihm auch immer wieder, dass jeder Fehler macht, aber ihre Worte scheinen keine Wirkung zu haben. Er beginnt, Ryan täglich zu besuchen und klammert sich an die schwache Hoffnung, dass die Lähmung vielleicht nur vorübergehend ist und von einer Quetschung des Rückenmarks herrührt. Docs Gebete werden eines Abends erhört, als Ryan seine Finger wieder bewegen kann. Kim und Doc behandeln einen Junkie, dessen Freund behauptet, sie sei bei einem Sturz verletzt worden. Als ihnen klar wird, dass ihre Verletzungen durch eine Schlägerei verursacht wurden, flieht der Freund.

After Jimmy tells him that the fire was probably deliberately set, Bosco is convinced that he saw the arsonist in the crowd outside after the fire. Sgt. Cruz gets Bosco and Faith assigned to the Anti-Crime Unit for a few days so that they can track the guy down. At the scene of another supermarket fire the next night, Faith takes down license plate numbers, Cruz photographs the crowd and cars at the scene, and Bosco nabs the man he saw in the crowd the night before. The man turns out to be a fire buff, but Faith and Cruz's work bears fruit and they find their arsonist -- the son of Doug Maple, an NYFD arson investigator. Emily discovers just what Faith's job entails and how much her mother loves her when they are taken hostage during a bank robbery, and Faith risks her life to save her daughter.

Monty and Joy hook up. Miguel witnesses one drug dealer murder another, and becomes the dealer's next target. On his first day back at work, Sully rides with Faith, who is put off by his short fuse and bad attitude. When he leaves a prisoner unguarded and uncuffed, Faith tells him to go home because he's not ready to be back on the job. Sully gets drunk and goes to his mother's nursing home. Sobbing about Tatiana, he tries to batter down the door when the receptionist won't let him in and calls the police. After Hancock arrives on the scene, Sully mouths off to him about Nancy in front of the other officers who are there to take him home.

Bosco is dismayed as he watches Cruz go to the extreme lengths of planting drugs, frightening children, and faking a dying declaration in their attempt to arrest the drug dealer who killed another dealer and then tried to to kill the 12 year old boy who witnessed the crime; Nancy tells Ty that she's ending her relationship with Hancock, and gives Ty the ammunition he needs to get out from under Hancock's thumb and back to duty at the 55, as well as getting Sully's suspension from duty lifted.

Over her protests, a judge marked for execution is placed under a protective detail and gives Sully and Ty a scare when she slips away to get a cup of coffee; Levine uses an unorthodox way to get a patient's mind off his pain; when no physical reason can be found for Faith's intense pain, her doctor refers her to a psychiatrist; Kim is livid when Doc gives her an official letter of reprimand for disregarding field regulations just the way he did several times; in an effort to get out of the bag and off the street, Maritza convinces Swersky to let her set up a sting operation to catch a chiropractor who's fronting for prostitution, and pays a terrible price for continuing her ""lonesome cowboy"" approach to apprehending criminals.

Internal Affairs drops the bomb on the envelope-pushing Detective Cruz as she is taken away and struggles to survive in jail, while Captain Finney drills Detective Yokas for information that will nail Cruz - but the torn cop secretly knows that she pulled the trigger on a career criminal that frames the case against Cruz. Officer Sullivan reveals more about his running feud against Finney and his rookie son. Carlos and Levine keep getting called to help a suicidal woman. Bosco's condition changes.

Ty rats out Sasha to the squad after Finney refuses to transfer her out; an action star doing research for his next movie goes on a ride along with the firefighters, and Stu quickly discovers that his boyhood hero is a failing alcoholic looking for a chance to revive a dead career; Brendan offers to ride with Sasha when everyone else refuses to do so, which leaves Ty and Sully partnered again; the retired detective who gave Sasha the real I.A.B. file on the death of Ty's father is found murdered by a police-issued bullet; Sully decides to testify against Finney; in pursuit of a drug dealer who shot an eight year old girl, Sasha and Brendan are injured, but the other units in the sector fail to respond to their calls for help until Ty and Sully finally relent and come to their aid.
